Nikolaus Gravenstein is Professor in the Department of Anesthesiology and Department of Neurosurgery, University of Florida College of Medicine, Gainesville, FL, USA. David A. Paulus is Professor in the Department of Anesthesiology, University of Florida College of Medicine, Gainesville, FL, USA.
